Steps for Centers Applying to Join the EURO-DM-CRN

Step 1

Expression of Interest

Centers submit a short online form to indicate their interest in joining the Network.
This first step serves to register your center and to receive the Feasibility / Site Readiness Form.

Step 2

Completion of the Site Readiness Form

Interested centers download and complete the full Feasibility Form, providing:

Basic institutional and contact information

Clinical activity and annual patient numbers

Availability of core assessments and required instruments

Staffing, dedicated resources, and multidisciplinary capabilities

Commitment to data quality, standardized assessments, and harmonized protocols

The completed form should be returned by email to the Network’s Scientific Coordinators.

Step 3

Review by the Scientific Coordinators and Scientific Committee

Each submission will be evaluated according to predefined minimum criteria, such as:

Minimum number of DM patients regularly followed

Availability of core assessments (HHD, spirometry, 6MWT, etc.)

Presence of a dedicated physiotherapist

Ability to ensure reliable data collection and quality standards

Eligible centers will be included in the first group of Network sites.
Additional centers may be placed on a waiting list, depending on capacity and future phases of expansion.

Step 4

Confirmation of Admission

Approved centers will receive:

A formal confirmation email

Information about their status within the EURO-DM-CRN

Instructions for onboarding and next steps

Details on data harmonization procedures

Step 5

Onboarding and Access to Tools & SOPs

Once admitted, centers will receive access to the Network’s shared materials, including:

Core dataset and data collection guidelines

Standard Operating Procedures (SOPs)

Templates and operational tools

Training materials and onboarding documentation

Instructions for data submission and participation in Network activities
